Ten years ago, billions of gallons of toxic sludge spilled from a plant in Tennessee and set in motion a chain of events that would lead to first-ever federal regulations on the ash produced from coal-fired power plants. 

Three years ago, those regulations were implemented. 

Four months ago, results of monitoring required by the rule itself were released, proving that nearly all coal ash ponds around the nation are leaking toxic pollutants into groundwater.
