I have struggled with this very question for many years. After countless hours researching internet blogs and grammar books, I only became more confused causing small panic attacks whenever a dreaded list and commas presented itself in sentence structure. I tended to simply leave it out to be red-marked later or just rewrote the sentence.
Finally, the age-old question was given some clarity and became case law when the United States Court of Appeals for the First Circuit weighed in on the matter last year.
